All form fields showing in Dreamweaver but not when uploaded to server

All form fields showing in Dreamweaver but not when uploaded to server

Couple of problems here.

1. The form looks like it should in Dreamweaver, but when I post it to the server it is completely different. Any ideas?

2. How do you get rid of that "PLEASE ENTER YOUR......." beside the form field. I tried just deleting it in Dreamweaver, but that doesn't seem to work.

3. How do you get rid of that annoying little box around the form?

Most likely the SpryAssets folder has not been uploaded to the server. make sure the SpryAssets folder is on the server, the display of the error mesafges is controllde by JS files ion that folder.

The box around the form is created by the field set tag. you can remove it by deleting the <fieldset> and </fieldset> tags on your page in Dreamweaver.
