Problems with Captcha

I have just finished yet another contact form using Captcha. I have many sites using the Captcha facility as provided by the Contact Form Solutions Pack.
Occassionally I have received feedback from my clients stating that people completing these forms are unable to get past the Captcha field. However, with this particular client the problem was more concerning.
I had tested the form on both my testing server and on the production server and using my browsers, all worked fine. I tested using XP Pro / IE 8. Also FF 3.5.2.

Now, here is the rub: My client also has XP IE and FF and was unable to get the form to get past the Captcha at all ! And to make matters worse, they started to get complaints from site users of the same issue. The following is an example feedback from a user:

Also, just so you know, the security code section of your website does not seem to be working, I tried three times to enter the code and it kept saying it was incorrect.

So I would like to know what can cause this. Are there browser restrictions ? Eg: Must have cookies enabled - etc etc....

Yes. Server cookies (session variables) must be enabled and functioning. That is the only restriction.
