After installing with Dreamweaver closed, you have to open dreamweaver and open any page in a site. That should initiate the secondary installer which will run to finish installation.

Did you see the secondary installer and wait for the 3 steps to complete?

After that you need to close and re-open dreamweaver before the menu items will work.

It sounds like either the secondary installer didn't run or you haven't restarted after. If it still doesn't work after restarting Dreamweaver, try reinstalling the extension and see if the secondary installer appears after opening a page.

Post back here with the results and we can figure out if we need to do a screen sharing session in order to debug further.

Please post a US phone number or Skype contact information. I'll set up a screen sharing session for Monday to investigate why the installation is not completing properly and get it working for you.
