Form does not send email

Hello Jason / Ray,

I am trying to create a form using Form Builder: set up the fields, validations, styles and for the form to be emailed.
However, when loading the site live and submitting the form as a test, it does not send it.

I set it up so that it posts to another php page, but instead it reloads the same page prompting that there was a validation error (even thought there is none.)

I trying doing it all over and deleting all dependent files to make sure there weren't any corrupted files and starting all over from scratch. but I still get the same error.

The page the problem occurs is in "request-appointment.php"

I can't seem to access that .zip file... please just attach the two php pages you are working on (the one with the form and the action page you post to) and I'll take a look to see if I can spot the problem.

If you include a URL where I can see the problem that would help, and if you want to give me FTP access instead of attach the files I could fix it on the server directly.

Hi Ray,

Thanks. Here attached I'm including again the 2 php pages in question, as well as the site again, separately, just in case.

It looks like your captcha field name got updated... potentially when editing the captcha design.

update line 344 of request-appointment.php and change field=security_captcha_2 to field=security_captcha

I'd also update line 5 of appiointment-request-submitted.php to:
$WAFV_Redirect = "request-appointment.php?invalid=1";
