Two Forms One Page - One does not insert record to database

My page has two forms. Both were setup with the same steps. Both have different names and the buttons are identified separately by name as the trigger for their respective form. For some reason the second form will not insert a record. Can you please take a look and tell me where I went wrong?

neither form should work do to the naming convention of form elements you are using.

In your form element names you are using the - character which is not a valid. For element names should contain only letters and numbers, the only exception to that is the underscore

camp-term

is not a valid name, but
camp_term

would be.

Thanks for the info on the incorrect naming. I changed all of those and recreated the server behavior but that still didn't work. After some tedious trouble shooting I discovered that one of my database fields had an attribute of "unsigned" for some reason. Thanks again for the help! The more practice I get with the Webassist software the more I like the possibilities!
