Email Password Help

I have a login page with User Name, Password and Submit button. I added "forgot password" text. Where do I link this text for a user to get an email with his id and password reminder?
Also, I added Security Assist Email Password behavior to the page.
Under Database tab, in "look up value" field, I entered "email". I am confused with what goes in this field.
Under Email Objects, I choose "mail" in the first field, and then left the rest as is.
The password is not encrypted.
Please help

The security assist wizard will generate the Forgot password page for you when it creates the login and registration pages. It is better to use the forgot password page that the wizard generates.

I didn't use the wizard to create the login page. How do I add it now?

In the look up filed, you should click the lightning bolt icon and select the email form element.

even if you are not going to use the wizard generated pages, it would be a good idea to have a test site that uses wizard generated pages, that way when you are trying to create them manually, you have something to refer to. You can look at the wizard generated page, to see how the behavior should be set up.

I used the wizard generated page for "forgot password". It worked initially, but now I have an error message coming up.

I included the webassist files as well as the forgot password file, so you can take a look.

What version of Data Bridge do you have installed?

Make sure you have the latest version, 1.1.4.

if you have a previous version, make sure to uninstall it before installing the newer one.

after installing the newer one, edit the forgot password behavior to have the code regenerated.

on the my account page, in the downloads section, click the All Downloads link, there is a button to clear activations on the all downloads page.

I uploaded a new version of Data Bridge, but I am still getting the same error message.
The id and password is emailed to me, but I am still getting the error :
