Form builder - header already sent

hi there

have added a contact form to my website, but having issues when attempting to send.
once filled in and click send, page returns with little messed up page :(
form gives error header already sent.
you can see and test it here: jamieoates.com
I did remove the line 83 from the wavt_scripts_php.php which is the only line I find which sends to header, but then still receve the same error, I have replaced it again incase but realy i need this working.
Any suggestions please?

this error is caused by output between php tags.

the error is pointing to line 67 of the index.php as the cause of the problem.
output started at /home/bogstench/jamieoates.com/index.php:67

Thanks Jason,
I found that the error provided ie line is about as helpfull as a dead duck, the index page is all html, so why theres an error on line 71, 70 or even 67 is all due to a previous include and not the actual html content on those lines.
I would suggest to add to the other post you mentioned that one should also check previous includes as they could also be the issue, especialy when there is no php code on the lines provided.
Thanks anyway, I basicly gave up now and scrapped the tracking script i had which was the real cause (included on line 30).

if the included file has a session_start() or header() function on it, that would cause the problem.

Basicly, the error occurs if either the session_start() or header() functions are used after there is any output on the page.

if your include file comes inside the html body and uses either of these functions, you will get the error you posted.

God help me please this has been driving around the bend.

So I have gone thru all lines of code I have, think im going blind :)
But anyway no white space to be found, have recreated the form a few times (due to issues with the generation of the form).
So now I have a new form added and still get the same error.
The only include I have within the page is the connections.php obviously for the sql.

The only thing I can find that could possibly be the issue is this js:

could it be possible that this is the issue?
Really am at an end here and obviously want to get this working, please help my sanity.
If it is the js above any ideas how i solve it?

Also have another question other than this, Im having problems finding where I can set a thank you for the mail message with the super suit, as when i send a mail i just get the same page back which is fine but nothing saying yes it worked (other than the http:// which is really lousy)
