Form builder - header already sent

God help me please this has been driving around the bend.

So I have gone thru all lines of code I have, think im going blind :)
But anyway no white space to be found, have recreated the form a few times (due to issues with the generation of the form).
So now I have a new form added and still get the same error.
The only include I have within the page is the connections.php obviously for the sql.

The only thing I can find that could possibly be the issue is this js:

could it be possible that this is the issue?
Really am at an end here and obviously want to get this working, please help my sanity.
If it is the js above any ideas how i solve it?

Also have another question other than this, Im having problems finding where I can set a thank you for the mail message with the super suit, as when i send a mail i just get the same page back which is fine but nothing saying yes it worked (other than the http:// which is really lousy)
