CSS Form Builder Error?

The site is hosted at GoDaddy on a windows platform. I had the company change it over to Linux. I'm going to guess that they might have to turn PHP option on? Not sure if that's automatic there or not.

Just checking... have you created a Universal Email behavior already?... or some other server side behavior to process the form and email it? If you have done that already, post the code so we can see if there is an error in your processing code.
