Add a new TextArea field?

The text fields that you have added look like they should be working to insert to the record. For the checkboxes you added you neglected to specify a value, a checkbox will post what is specified for the value if it is checked so you should make sure you update this part.

Rite now on line 172 you have the code to execute the query, you should add in a die statement to see what is in the query like this:

from this:

php:
172: $MM_editCmd = mysql_query($WA_Sql, $WA_connection) or die(mysql_error());

To this:

php:
die($WA_Sql);

$MM_editCmd = mysql_query($WA_Sql, $WA_connection) or die(mysql_error());

This will get the query to print to the screen, examine this query to see if you can spot any problems, if not get back to us with the results you get. Also, if you unsure if the query is formed correctly you can run it directly from within your MySQL GUI tool. Please also let us know if any part of the record is being inserted.
