I finally figured out how to make it work. I had the Value still at Value in the drop down menu options. So I changed them to Yes/No and now it emails me the correct... stuff.

So that's fixed. :)

Now another issue, on another form, I have it set so it emails me and the person who fills out the form as the CC. But, when it emails to the CC address, it's a blank email.

Any ideas?

Does that happen to any email address, or just a particular one? Have you tried sending via the Recipient and/or BCC field?
