Build Form / Universal Email allow files uploaded by client

I am in need of two simple modules. We don't really develop much these days,

I need a login script that sends an email for a double-authenticated message. I can do it manually but don't have the time. Quite literally. I need to move fast!

I also need to build a form with mandatory fields that prompts AND prompts for JPG, GIF, PDF, etc. The form needs to be self-validating and able to send emails that it was successfully completed or an error occurred.

Can you help? This is for a fast moving non-profit organization.
