Creating questionaire that accepts multiple choices

Hi,

I am new to WA extensions (Super Suite). I wish to create a questionaire form that contains questions that allow multiple selections (check boxes) and when submitted feeds a SQL database and sends email letters to the submitter and myself.

What's the best way to handle the questions that allow multiple selections. Both ith the form and with the database?

Once I decide how to handle the multiple selections, in what order do I use the WA extension?

Do I begin with CSS Form Builder? How do I get the information into my database? and set up the email letters.

The WA extensions users guides just explain the particular extension, they don't discuss tieing the tools together.

The tools work in a modular fashion, you just put together the tools to create the experience you are looking for.

For the first part of this you would use CSS Form Builder to create your form. Once it has been created you can then add in a DataAssist insert server behavior to ensure that the information entered by the user will be inserted into your db.

To get this information sent in an email message you can apply Universal Email to the page. This will create the server behavior that sends the information in an email message. You should apply them in this order. Other than that there is nothing that is specific about the way that you will apply the server behaviors to achieve your result. Eric,

You complete passed over all my references to... how to handle form fileds (questions) that allow multiple check-boxes to be selected. How should I set up the database.

Do I set up the database so each question is a row (record)? Can the insert record behavior insert multiple check box responses (for the same question) into a single record

or

Should I setup the database where each checkbox response is a row in the table?

My apologies for skipping over your multiple selection questions. If you have multiple checkbox selections you should have a column for each selection in your table. This way you can make a very simple association between the checkboxes and the columns within the DataAssist insert server behavior.

The other way to do this is to store an array of values into a single column in your db. This method requires some additional php coding to work out though and the values are harder to get at and search through if they are all stored in the same column.
