Creating questionaire that accepts multiple choices

Hi,

I am new to WA extensions (Super Suite). I wish to create a questionaire form that contains questions that allow multiple selections (check boxes) and when submitted feeds a SQL database and sends email letters to the submitter and myself.

What's the best way to handle the questions that allow multiple selections. Both ith the form and with the database?

Once I decide how to handle the multiple selections, in what order do I use the WA extension?

Do I begin with CSS Form Builder? How do I get the information into my database? and set up the email letters.

The WA extensions users guides just explain the particular extension, they don't discuss tieing the tools together.
