email not sending

Hello all,
I've made at least 10 website using the same method and syntax but the last one is not working as expected.
Basically I have a form collects some data and then submits the data to a database, sending an email.
Now no email will be sent. Why?
Obviously I'm in a hurry and I have tried everything to make it work. But no email will be sent from this site.
I'll upload the not-working page here.

I hope someone can help me.

Please consider that the email log is stating the email has been sent. And that the record is regularly inserted in the database.

What else can I do?

Can I get FTP access? I'll have to play with the code to debug it and see the include files. Running the live page doesn't do me much good.

I don't see a problem in the code on the page you sent, so perhaps the problem is in one of the includes.

I'd start by dumping the session on the success page and see if it looks like it thinks it sent correctly:

perhaps I narrowed the problem: DNS.
I was using as sender a 3rd level subdomain email (info@mysite.mydomain.com) which DNS are not set already and so they don't resolve correctly..
I discovered that once I've set up the email account on my pc.
Now I used another temporary address (that resolve DNS correctly) and the email are going out correctly.
Sorry for the inconvenience.
