email not sending

Hello all,
I've made at least 10 website using the same method and syntax but the last one is not working as expected.
Basically I have a form collects some data and then submits the data to a database, sending an email.
Now no email will be sent. Why?
Obviously I'm in a hurry and I have tried everything to make it work. But no email will be sent from this site.
I'll upload the not-working page here.

I hope someone can help me.

Please consider that the email log is stating the email has been sent. And that the record is regularly inserted in the database.

What else can I do?
