Dreamweaver CS4 crashing when using CSS Form Builder

Hello. Hoping you can assist. I have read through related messages for help, and after trying a few things, I still have my Dreamweaver crash when customising fields on a new form. CSS Form Builder just locks up and says (Not Responding).

Problem first started whilst I was using version 2.0.0. I would get as far as clicking the 'Edit' button from the Form Contents screen, and it would lock up. I discovered a new version, so uninstalled the earlier version, and installed the newer version. Software appeared to run a bit quicker and I was able to add and delete fields, so I thought the issue was resolved.

I don't even get as far as finishing the form before it no longer responds. I can pre-select a form, and edit the fields and go back to the Form Content screen, but if I try to re-enter the Edit button or Finish the form, it stalls, and says (Not Responding).

I have cleared the .dat file, closed and restarted Dreamweaver, created a new php page, closed down the majority of my programs to free up memory. I don't know what is wrong. I have relooked at the 'common problems' page and done its suggestions. Problem is still occurring.

Something strange is happening also, the form I saved as a single preset, the preview has applied this design into every preset form, and all the form designs look the same. This is a mess.

It has been a while since I have used CSS Form Builder, but on last time, I haven't had this issue.

I use Dreamweaver CS4. Windows 7 PC.
CSS Form Builder 2.0.1, this is the only Webassist extension I have installed.

recreate the users configuration folder as outlined in step 12 of the Adobe troubleshooting guide:

then uninstall all extensions from the extension manager, then reinstall form builder:

Hi Jason, thanks for that. I did the Dreamweaver CS4 regedit reset. I was hopeful.
Went to create a new form, got as far as selecting the drop down of 'Registration' template, and it's locked up again.

Disheartening... now i really don't know what else to do.

Hi. I've been trying the forms again, and I've had more success without it locking up. I've disabled the automatic preview and I think this has helped.
So I've managed to implement a form into my .php page. I then go to create a email message. Now I hit Javascript errors.
First one that pops up is relating to WAUEInsp.html (have attached a screen shot). This repeats 4 times.
Second one that pops is a validation rule error. I have not changed any code of the form, and the name is still is a required field, but I get an error relating to this now. On either clicking yes or no, my php page is completely ruined with all formatting disappearing, and it looks like a mess.
I've shutdown Dreamweaver, deleted the WinfileCache.dat file, and restarted. I get another javascript error saying wa not defined...

Pleeeease help! I appreciate your time.
