Dreamweaver CS4 crashing when using CSS Form Builder

Hi. I've been trying the forms again, and I've had more success without it locking up. I've disabled the automatic preview and I think this has helped.
So I've managed to implement a form into my .php page. I then go to create a email message. Now I hit Javascript errors.
First one that pops up is relating to WAUEInsp.html (have attached a screen shot). This repeats 4 times.
Second one that pops is a validation rule error. I have not changed any code of the form, and the name is still is a required field, but I get an error relating to this now. On either clicking yes or no, my php page is completely ruined with all formatting disappearing, and it looks like a mess.
I've shutdown Dreamweaver, deleted the WinfileCache.dat file, and restarted. I get another javascript error saying wa not defined...
