Email not sending

This form is not sending emails http://carolinamedical.us/contact-carmed.php. None of us are receiving the three emails I have inputted, one of which uses the domain name in the email address of the website - carolinamedical.us

Who is your host?

What have you used for the From Address? Many host require that the from address be a valid email address for your domain.

Make sure the from address you use is not the same as any of the To addresses.

Some hosts will require that you select the option to include the From address as an extra parameter.

What did you enter for the SMTP Server address? Some hosts will require that you enter the SMTP Server address.

there are apparent bugs in this program.

1. Every time I make enable the check box to send by email, it is gone the next time I check on this.
2. It erases the email I put in each time I enter it.
3. It changes the columns in the form each time I make any change.
4. It will not save a design theme form me - I have to reenter this each time.

1 - 2) It is not supposed to retain that checkbox on editing the form. To make changes to the email settings, you need to edit the universal email server behavior that is added to the server behaviors panel.

3 - 4) Please provide more details.

The form preset does not retain the input I provide. So, the preset for the theme changes from what I input. So, I have to redo this every time I make a change. The main issue, though it that it does not send the form results to the email.

I tried putting an actual email in the from field, but that doesn't work.

Where can I change the smtp. There is no field for this? I have spent many hours on this like all your extensions, they are buggy. What next please?

Open the server behaviors panel, and double click the universal email server behavior to edit it, click the Envelope icon to edit the email server settings.

send a copy of the contact-carmed.php file please.

also, in my initial reply, there are a number of questions that i asked that have gone unanswered.

I need to know this information in order to help solve the problem.

since the email settings are unique to your server environment, i need to know details about your server environment.

I have used an email not used in the To field, and I have used one generated from the email field. I have used an email from the domain of the host also. - None worked
What is an extra parameter and how does that work?
mail.carolinamedical.us is the smtp
hostmysite is the host which is at hosting.com
attached is the file we are using.

I'll also need a copy of the webassist/email/waue_contact-beta_3.php file please.

In the Universal Email server behavior, Click the envelope icon to go the email Server settings screen. there is a checkbox on this screen to Include the from address as an extra parameter, some hosts require this option to be selected.
