form builder confusion

I have just installed Design extender and data bridge to DW CS6.
I want to add a simple form to register a download and add captcha. Add email form details to user and myself.
Ive done all this prior with DW forms and UE4.01
Form Builder so confusing.
I keep getting to chose from preset forms like login and password etc.
I cannot get past these presets.
How do I create a simple form without using the presets. Help tells me I can do it but offers little "how to"
Also
How do I add captcha to an existing form. I tried the insert button but nothing happens.
How do I refresh the captcha code without refreshing the browser, I could do this in the old version
Any video tutorials regarding Form Builder?
Thanks
David

If you keep the first list selection at "new from blank" then click the pencil icon after the second list, you can build an email from scratch and save it as a preset when you are done.

In your case I'd probably still start with the preset "Contact", "Simple Contact" and add and remove fields using the pencil icon from there.

Form fields can be added in groups and groups of form fields are organized into lines. Each line can have up to two form fields and each group can have any number of form fields. The process is a little over-complicated since we really wanted a lot of control in the form, and unfortunately the usability did become compromised by the complexity of the control. We will likely add a "simple" mode to allow basic form creation easier in the next version, but hopefully my description here will give you enough to get through it without too much difficulty.

to add captcha to an existing form, the page must be a dynamic PHP page. it will not work if the form is on an HTML page.

see this post for details on adding a refresh link to the captcha:
