form builder confusion

If you keep the first list selection at "new from blank" then click the pencil icon after the second list, you can build an email from scratch and save it as a preset when you are done.

In your case I'd probably still start with the preset "Contact", "Simple Contact" and add and remove fields using the pencil icon from there.

Form fields can be added in groups and groups of form fields are organized into lines. Each line can have up to two form fields and each group can have any number of form fields. The process is a little over-complicated since we really wanted a lot of control in the form, and unfortunately the usability did become compromised by the complexity of the control. We will likely add a "simple" mode to allow basic form creation easier in the next version, but hopefully my description here will give you enough to get through it without too much difficulty.
