Attachments not being sent 4.0.1

hi there

I have UE 4.0.1 and the email content is being received but no attachments...

I have the attachments set as 'Submitted Files' as you'd recommended elsewhere.

I've attached my main page plus the UE email page

the submitted file option only works if you are not using the File Manipulation -> Upload behavior to save the files on the server.
