Apply Login/Register functionality not working

Jason,
After going through the pdf tutorial on "How to Apply Login/Register Functionality to your Checkout Process" several times, I am still unable to get it to work. I seem to be having problems every step of the way.

I did find an earlier thread where I made a similar mistake in the value field (Modify > Security Assist > Access Rules Manager), where I typed in "checkout" instead of going through the lightning bolt to make the selection. It would save alot of time if the instructions would make this clear. I have adjusted this mistake, yet I am still allowed to continue to checkout form my cart page. I am attaching a zip with my LogIn, Cart, Checkout, and Confirm pages. All my login pages were created through SA1.

I also have an outstanding Forgot Password issue on the following thread:

I really need to get this website live. Please help!

Now Show Region not working

The apply login functionality is working now, but I do not get the "show region" text applied to the LogIn page.

Session Ticket

Thank you Jason! I have responded to the ticket session.

For now, I have just created a second LogIn page for the checkout redirect, since I could not get the "show region" text to show correctly. But now I have another problem. In uploading my files live, the registration page is not being redirected to either the LogIn.php page (successful) or the ExistingAcct.php page. It goes to error, "The webpage cannot be found" HTTP 404 Not Found", similar to your correspondence in thread showthread.php?t=20916&highlight=registration+page+live. I have checked the helper file, as was mentioned, and it looks correct. I have recreated the page, adding a username field and am still getting the same error. I am attaching my registration.php and the helper page file, just in case you would like to look at it before the ticket session.
