CSS Form Builder

I've created my form, and when I use the Universal email extension, I'm getting confused on the last step which is designing how the form will look when I receive it. The two options are: design with Dreamweaver and HTML. I'm a novice at Dreamweaver and I don't know how to finish this last step. Help!! (Please)

What version of Universal Email?

If it is version 4, select the option to create a file for the email message and select from one of the templates.

Form Send To

Thanks for your response. I've upgrade to version 4. I should have mentioned that I don't know how to link the "Send To" email to the Submit button. I appreciate the help.

to use a form value for the send to address, click the lightning bolt next to to address, then select the corresponding form element from the bindings panel.

Not sure you would want to use the submit button, unless the submit buttons value where set to an email address, but you could use a text box and allow the user to fill in thoer email address.
