Cannot install MenuWriter 2

Hello Mr. or Mrs.
I used to work with MW 1.0.4! Recently I bought MenuWriter 2.0.3. When I install this version with Adobe Extension Manager CS4 everything works fine and Adobe tells me that MW 2 is installed correctly. But after starting Dreamweaver CS4 version MenuWriter 1.0.4 appears!!
When I remove the Menuwriter extension complete with Abobe E.M., after starting DW CS4 still the Menuwriter 1.0.4 appears and can be used!!!
How can I remove MW 1.0.4 in order to install MenuWriter 2? With Adobe Extension Manager this seems impossible!

The Configuration folder may have been corrupted. Try renaming the configuration folder and see if that resolves the issue. Here is a post that goes over how to do that:

The first part of my problem is solved. But now the second: After removing MW 1 I installed Menu Writer 2.0.3. The Adobe extension manager tells me the CSS MW extension is correct installed. However, when I open Dreamweaver there is no Webassist Menu Writer in the Insert menu or toolbar, and also not in the help button! I tried it for three times, but without succes. How can I make CSS Menu Writer 2 get working?

I Hope you will answer my guestion above as soon as possible, because at this moment I can not use Menuwriter, version 1 or 2, anymore.

We were able to talk on the phone and determined the issue being that the install was going to a nl_NL directory instead of the en_US directory, so copying over the configuration from the nl_NL to the en_US directory caused the menu items to show up.
