Labels not showing in Security Assist user update form
I have a User Update form that does not show any labels. The code is as follows for the first label in the group of 6 fields:

This form is created by the security assist wizard, and the only changes that I have made is to reduce the length of the built in error messages so that they don't spread across a 22 inch screen when displayed. The text should show up and I cannot figure out why it does not. Unfortunately the code is live on line, so I cannot post a link to the page, and it has 16 associated files, and is protected.
Suggestions as to how I solve this problem would be welcome.
