Javascript errors when using multiple insert behaviour

I've been trying to use the multiple record insert behavior using data from a drop-down list as the data to be inserted. However, every time I click on the option to add the behavior I get the following Javascript error:

'While executing CanApplyServerBehavior in WADA_MultiInsert.htm, a Javascript error occurred.'

I've tried clearing the dreamweaver cache and I've also tried it on two diffferent machines and both result in the same outcome.

I've attached a copy of the page where I'm trying to add the multiple record insert. I'm using Dreamweaver CS5.5 on PC.
