Javascript errors when using multiple insert behaviour

I've been trying to use the multiple record insert behavior using data from a drop-down list as the data to be inserted. However, every time I click on the option to add the behavior I get the following Javascript error:

'While executing CanApplyServerBehavior in WADA_MultiInsert.htm, a Javascript error occurred.'

I've tried clearing the dreamweaver cache and I've also tried it on two diffferent machines and both result in the same outcome.

I've attached a copy of the page where I'm trying to add the multiple record insert. I'm using Dreamweaver CS5.5 on PC.

you must us a Repeat Selection for the multiple record insert behavior, it cannot be used with a multiple select list.

I'm not sure I understand the difference - could you clarify? I picked the table row as the repeat selection and then clicked on the insert multiple record option and got the JavaScript errors.

The multiple insert is triggered from hidden form elements that get created for each text box in the repeat selection.
