1-2) What version of Universal Email are you using? Can you send a copy of your page and the webassist directory so i can look at the code.

looking at the source of the page, you have form elements that are using a space in the name:
<input type="text" size="60" id="Email Address" name="Email Address">

this is not valid, form element names cannot contain spaces.

3) In the bindings panel, there is a binding group name Validated Entities. This group contains the valid posted values.

Set the initial value for each form element to use the corresponding binding from the validated entries collection.
