Since we have the same version of the extension installed and are getting different results with the same page that tells me that there may be an issue with your installation. The configuration folder may have become corrupted, you can rename it to fix this then remove the installed extensions and re-install them. Here are the specific steps you will need to take to do this:

If you complete these steps then reinstall Form Builder and have these issues still let us know and a ticket can be opened for you on this issue.
