A couple of things to start diagnosing the issue...

One, I would definitely use "Post" in this situation rather than "Get". The Get method sends the variables through the URL Address which is very insecure.

Also, when your form starts to process, it tries to go to "photoform.html". I am guessing you would want the processing stuff on the same page as your form which is "photoform.php". Note - the extension difference (PHP as opposed to html). So you will probably need to correct that first.

The next question to help you get going is what do you want the form to do? Do you want to populate fields of a database or, instead, email someone with the inquiry?
