Email is not being received by indicated persons

technically (a) and (b) are the same ---

1) But something strange is happening. When I completed the Free Consultation Request on (a), it gets sent and sent message is received by designated person

2) When I completed the Free Consultation Request on (b), it doesn't get sent and sent message is not received by designated person

Q: does anyone know why this is happening?
