I don't see any problems with the page. You may have to contact paypal support about this. I found that it does work if I enter a different address like:
223 E. Concord Street, Orlando, FL 32801
It appears their system isn't recognizing some addresses and it does address verification. I wonder if you could turn off address verification from the manager portal? That might be a solution. I'd contact paypal support and give them the example of the address it works with and the one it doesn't and hopefully they can point you in the right direction. It doesn't appear to be an issue with the code on the page.
