seems perfect on my localhost.
I added few more insert records & redirects perfectly for all in my localhost. But doesn't work on my server.
On Clicking button : <input name="Continue" type="submit" class="btn btn-md btn-motive" style="font-size:14px; margin-top:20px" value="Continue">
it must redirect to next page ie confirm.php but it doesn't redirect to next page. i tried playing around a lot with other things like form action & etc. yet doesn't redirect.
it just redirects to same page booking.php..
