You have a bunch of repeated .js files on the page. And the jquery files need to be above some of the others that it is currently below.

I'd start by moving:
<script src="//"></script>
<script src="//"></script>

above the other script references and then remove any .js files that are repeated... These three particularly:
<script src="webassist/forms/wa_servervalidation.js" type="text/javascript"></script>
<script src="webassist/forms/wa_clientvalidation.js" type="text/javascript"></script>
<script src="webassist/forms/WA_ShowCVV.js" type="text/javascript"></script>

Then remove the final jquery reference on the bottom of the page:
<script src=""></script>

Once you make those changes if you still have issues post back and I'll look at the page again.
