I made a form and it is not working please help

Thanks Ray,
I got the ones at the bottum those where easy to spot, however i didnt write this code i really have no idea what im looking at.

Assuming your a code guy, im hoping i can attach pictures of the remainder of the code that i am not 100 percent sure of and you can point out what is the
-Jquery core js file
-what the code look like for googleapis.com reference?

Sorry man, i just don't want to cause more problems on this form for the future since i need this problem to go away, its important for my company.

Lastly i do not know if it does anything or not but i changed the name of the page recently, would that be a problem for the code?
