I picked Email_Address from the form persistence binding group to fill in both the From: and the To: fields.

The form address and to address should not be the same.

the from address should be a valid email for your domain.

i though you where creating the recipe to send the email top the store owner, so you would want to use their address as well

I can't seem to find where to set the body to use a template though. The default is checked in the "Manage email body from file"...?

yes, select the option to manage he email from a file, this will create the template file.
