CSS form builder files missing?

CSS form builder files missing?

I am using CSS Form Builder to create a simple contact form.

In my test I am getting at the end of the email the following items that are not required.

"Hidden Field:
SimpleContact submit: Contact me
Additional Notes: Lorem ipsum dolor sit amet, consectetur adipiscing elit. Donec eget tellus sed justo rhoncus posuere id sit amet arcu. Morbi pretium, enim faucibus facilisis hendrerit, elit. Morbi quis sodales ligula. Pellentesque elementum faucibus elementum. etc"

If I select the page Form then I can select CSS Form Edit Form
When I select this I get three options.
Form Function looks like it should give some help but when I select any of the links in Form Funtion I get File not Found.
I am attaching an image of this

Am I some files missing
and how can I remove the above fields please?
