Error upon trying open Form Builder

Thanks I ended up upgrading to Adobe Creative Cloud and I reinstalled extensions using new didn't give me the preview error any more, and all seemed to be working, though I tried to use the email log feature and now when I submit the form I get this error: Table 'db515551049.email_log' doesn't exist

When I do open my site I get a message that it "needs to upload the Connection file" - even though it is already uploaded since the site was made in Dreamweaver CS4...I click yes, but I still have an "X" next to my connection files - not sure if this has something to do with it?

The emails submitted with the form do send...but the user gets the error message mentioned above.
