Ok, so it's an error in the shipping calculation.

how do you have shipping set up?

Arte you using one of the rate Lookup services like USPS?

if so, open the bindings panel, and expand the binding group for you rate lookup service and add the full request and full response bindings to the failure page, this will show the response from the lookup service to help determine the cause of the failure.
