File upload not working

1) For File Upload to work, the forms encoding type must be set to multipart/form-data.

In the tag selector, select the form tag and change the encoding type to multipart/form-data in the property inspector.

2) You are using the Dreamweaver Insert behavior. The DW INsert Behzavior is limited in only being able to use form data bidnings, it cannot access the DFP Bindings that are created, you should use the data assist insert record behavior instead.

3) The File upload code happens after the insert code, it should come before the insert code.

  here is the file,

but I don't know which binding is correct:

-Client File Name
-Server File Name
-Server Simple File Name

The Server File Name binding is the correct one to use. That binding will contain the files name that is created on the server.
