more errors with this program

I am now getting the following error when I try to fill in the form function contents: "Please select some contents for your form before." I select items repeatedly, but no change.

Also, the form is not going to the confirmation page I selected.

I am getting the following javascript error when I run the form builder: (see attached file)

Are you sure this program was tested before releasing it to the public????
