General Shipping question/issue

First, should you have a shipping quote on the cart.php page prior to clicking checkout and submitting your shipping address? I do and it's not correct.

Secondly, my shipping rates are wrong and all over the place and I am not sure where I went wrong.

Using the USPS API and from another post I ran this on my confirm page (assumed it goes under the BODY tag)
<?php var_dump($_SESSION);

and this is the output I think you need.

I'm not sure what other info you'll need. And as usual, any help is appreciated!
