for the errors:

go to Window -> Work Space layout -> Reset '<your layout name>'

and Delete the Dreamweaver Cache file from the users configuration folder:
common_installation_issues.php

for the problem with form builder, if you upgraded to the latest version of supper suite, you should only have 3 extensions now:
eCart
Design Extender
Data Bridge

Make sure that eCart was not the last one that you installed.
