UE in DW templates

I am trying to set up a simple sign-up form in a Dreamweaver 5.5 Template that emails. DW saved the template with the php extension, LandingPage3.dwt.php . I made a test file, test2.php , and applied the template to it. Does not send. I notice the UE Server Behavior has an explanation point in front of it so I fear I am setting this up to fail from the get-go but I have tried reconstructing UE several times and am stuck. I also pasted in a line of code per this UE wizard warning: You have added or changed code outside the <html> tag. This change will not be copied to documents based on the template unless you insert this tag in the <head> section of your template:

Also just installed Data Bridge
