hmmmmm, change:
<?php echo WA_DFP_AllUploadsDebug(); ?>

to:
<?php echo WA_DFP_AllUploadsDebug(); ?>
<pre><?php var_dump($_FILES); ?></pre>

and post back the results after testing upload again.
