Image upload used to work fine. Now not displaying image on page.

I have an Insert item form with image upload that has been working for probably well over a year but suddenly something is wrong. I didn't change anything in the pages. Image file seems to upload just fine. I can see it on the server but it won't display on page. Yet all other images display on page.

Has something changed on your end that could possibly cause this? Or was there an upgrade that I don't know about? I just can't figure this out. Is very strange.

Here is a page. You will see item 'atest' has no image.
