Forgot Password page missing argument 6 error

I used Create Security Pages to create a forgot password page. When I run it in test with a non-existent email it quickly comes back to say the email can not be found. When I run it with a valid email it processes for a long time then comes back with the following error:

Warning: Missing argument 6 for WA_SecurityAssist_Definition(), called in C:\server\www\\public_html\ajfs\forgotpassword.php on line 142 and defined in C:\server\www\\public_html\ajfs\webassist\security_assist\mail_php.php on line 34

How do I get the page to work?

I have already deleted the Dreamweaver CS5 Configuration folder and re-installed the Data Assist 2.08, CSS Form Builder 2.0.0 and Security Assist 2.03 extensions in that order.
