Help with my page :(

Hi,

I am doing a simple one field test page (attached) to see if the Universal Email works.
I filled the field, attached one txt file and pressed "submit".
It seemed successful and there was no error message but 30 minutes later the email has not arrived!
What could be the problem, need help :(

thanks in advanced :)

UPDATE: I put a confirm page to make sure that everything is working fine but got this error:

Warning: require_once(../../../webassist/email/mail_php.php) [function.require-once]: failed to open stream: No such file or directory in /data/17/1/23/25/1838025/user/1994420/htdocs/adman/adman2011/Adman_2_Lang/swe/testingPHP.php on line 1
