your form is redirecting to the thank you page, so the script is sending the email to the SMTP Server, but the SMTP server is refusing to relay the message for some reason.
Who is your host?
In the Universal Email settings, what did you enter for the SMTP Server?
Some hosts may require that the from address be a valid email address for your domain, what did you enter for the from address?
Some hosts may require that the from address be sent as an extra parameter, you may need to check the Include From address as additional Parameters option on the server settings page.
On the thank you page, add the following code for troubleshooting, just before the closing </body> tag.:

I have never have to specify SMTP server ??? not ever? At least not since I have been using UE. I have always indicated "To" emails.
My host is Bravehost.com
in some cases, you can leave the smtp server blank, that will cause it to use the default setting in the php.ini file. in other cases, you have to specify the smtp server, it is entirely dependent on the host.
In this case it becomes a bit tricky. This is because this websites is physically located on bravehost.com at the path contactus.php . There is a forwarding implemented where the domain www.praiseworshipprayer.com/ on another hosting service register.com is pointing to it on bravehost.com.
Q: Which SMTP server do I have to define to make UE work ---- the bravehost or the rigister.com ???
I would think you would need to point to the bravehost SMTP server, but you may need to experiment with both
Do you have a Gmail account? does bravehost support using the Pear Mail For SMTP Object?
if so you can use the Gmail SMTP server by setting the Email Object to Pear Mail for SMTP,
for the SMTP Server address enter:
ssl://smtp.gmail.com
set the port to:
465
enable to option to use encryption and enter your gmail email address as the user name and the gmail password as the password
1. I have added "<?php var_dump($_SESSION); ?>" without the quotations to the place just before the </body> tag on the thankyou.php page.

no, you can still send the email through the GMail SMTP server, that is show i test emails on my local system
